Home of Hope LLC is a New Hampshire-certified residential care provider serving adults aged 18 and older with intellectual and developmental disabilities who qualify for ID/DD Waiver Services. We specialize in small, structured home environments designed to provide individualized care, close supervision, and meaningful one-on-one support. Each residence is intentionally designed to serve a limited number of individuals, ensuring that every resident receives personalized attention, consistent support, and the opportunity to live in a safe, home-like setting. All services are delivered through individualized Service Agreements and Implementation Plans, tailored to each resident’s needs, preferences, and life goals.
